Your Chrysler Service Contract will be administered through Siemans Chrysler, Inc. Please fill out the information form below completely. A representative from our dealership will be calling you to verify your information, collect payment, answer any questions you may have, and sell you the service contract if possible. We do not sell service contracts and warranties on the Internet. All sales are done over and by the phone only. This Plan is a service contract between you and Chrysler Service Contracts Inc. The Plan protects you against major repair bills should a component covered by the Plan fail in normal use. Chrysler Service Contracts Inc. is solely responsible, (liable) for the fulfillment of the provisions of the Plan. Extended-Warranty-Pro.com, and Siemans Chrysler, Inc. are at no time responsible, (liable), for any fulfillment of the provisions of the Plan.These are Chrysler Service Contract and any warranty issues should be directed to Chrysler Service Contract, Inc. By Accepting this agreement, means the Plan and the Warranty Disclaimer, indicated above, have been reviewed by you, and if your application is approved by Chrysler Service Contracts, you accept its terms and conditions.
